Description	This project focuses on the importance of life-course vaccination for patients with chronic diseases. The overall objective of the project is to provide means towards increasing the vaccination coverage of such patients by increasing their uptake of vaccines. More specifically, the project aims to provide patients with chronic diseases as well as health professionals and supporting communities, such as patients’ associations, families and caretakers, with tailored information about the vital importance of vaccines in their specific context, thus improving their confidence in vaccines and empowering them to make the best possible decisions on vaccination.
