Where is the 'declaration of intent' form to be found, or is the tenderer required to produce such declaration itself? If yes are there any procedural/formal requirements?

There is no 'declaration of intent' form as such. You should prepare yourselves the declaration of intent, which should document the willingness of sub-contractors to accept the tasks and their acceptance of the terms and conditions set out in the tender, reflecting the work on the contract in question.
