In looking through Annex II, we do not see that there is such a form available. Are you therefore making reference to the Yes, No, N/A checkmarks in section VII-Selection criteria (5) (c)? Thank you,

The form to fill in and submitt is the Annex II itself. You should fill it in answering to each quesiton with Yes, No, N/A for all the sections, wherever applicable. It shall be then dated and signed by the legal representative.
