The extract of judicial record of authorised representative is to be submitted in addition to the one related to legal entity? If yes, this shall certify only the existence of open procedures or also past judgements? Shall it cover all type of offences (criminal, civil, administrative ) or only administrative?

Your understanding is correct. The extract of judicial record shall be provided for the authorised representative, as well as for the company, as a separate legal entity. The information included in a judicial record varies between countries and even between jurisdictions within a country. In essence the certificate shall confirm that the grounds for exclusion, as defined in annex 6 "Declaration on honour on exclusion criteria and selection criteria" are not applicable to the person/ company concerned.
