it means that in case when share of the work is less than 20% of the contract the contractor has not to complete and sign this identification form?

Not exactly, the 20% rule is just for subcontractors. Please be aware that ALL tenderers and contractors have to fill and sign Annex I, plus all SUBCONTRACTORS whose share of work is more than 20%
