On page 18 of the ITT, you refer to detailing services performed by sub-contractors, percentages and tasks. However, the table provided for references in Annex 4 does not include a column for this information. Where should this information be included?

This information should be within the "description of contract and of relevant services"; to be provided in the last column of the first table at the end of Annex 4 "Declaration on honour on exclusion and selection criteria".
